Primary Health Services Center (PHSC) is a private, not-for-profit (501(c)(3) clinic founded in 1997 by local citizens in Monroe, Louisiana. As a Federally Qualified Health Center (FQHC) for more than 20 years, PHSC is an organization that participates in the overall healthcare of the community and has expanded its services to include Ouachita, Morehouse, and Lincoln Parishes. PHSC has been recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH). PCMH).
