-There is an annual cap: many PPO plans have an annual cap, which means that there is a limit to the amount of dental expenses you can be reimbursed for in a year. If the cost exceeds that cap, you'll have to pay the rest out of your own pocket.

-There are deductibles and co-payments: Some dental services in a PPO plan require you to pay a portion of the cost up front and then be reimbursed on a sliding scale. Depending on the situation these costs may or may not add up to a lot, more for complex dental procedures for sure.

-Less reimbursement for out-of-network doctors: PPO plans also regulate out-of-network doctors, and the reimbursement rate is usually lower than for in-network doctors. You'll need to pay more out of your own pocket if you choose an out-of-network doctor.

-Understanding what's covered isn't easy: It takes a lot of work to figure out what PPO dental insurance covers, which doctors are in-network, which services are reimbursed, at what rate, and so on, and this can be a bit of a hassle for some people.
